Despite huge technological advancements, today's medicine is in a state of crisis, claims Lown (professor emeritus in cardiology at Harvard and cofounder of Physicians Against Nuclear War). Emphasizing that nothing can replace listening and careful history taking, Lown laments that doctors have now substituted technology for taking time with the patient, shifting their medical focus from healing the patient to curing the disease. In a teaching style reminiscent of Sir William Osler, Lown extracts from his 50 years of medical practice case histories and examples of the clinical wisdom that enable a doctor to comprehend essential medical problems. Despite his Oslerian call to return today's depersonalized medicine to the art of doctoring, he evinces little optimism that his wisdom will be heeded in this age of Medicare and managed care. Cardiologist Lown combines autobiography with a plea for thoughtful and individual medical care of each patient. Although he draws his case illustrations from 40 years of practice and research, his is no nostalgic voice reporting from some comfortable ivory tower. He vigorously opposed the traditional prolonged bed rest for heart patients, pioneered in the establishment of cardiac care units, invented direct current cardioversion, which has saved many lives, and made other valuable contributions. He was one of the first Western physicians to visit China and later benefited from acupuncture treatments. In 1985 he and a Russian physician, cofounders of Physicians Against Nuclear War, received the Nobel Peace Prize. Healing, he asserts, has been replaced in his time by treating, caring by managing, and the art of listening by technological procedures. He points out that malpractice cases are not the cause of what ails U.S. medicine but are instead a symptom of the real cause--failure in the art of healing. William Beatty
